U16 Makeup Tryouts: Wednesday, November 29th at Daniel Bagley Elementary

STEP 1:

PLEASE CONFIRM YOUR PLAYER’S CORRECT AGE GROUP PRIOR TO REGISTERING.

IMPORTANT:
Tryout fees are $40.00, payable by card at time of registration. Please make sure to register as soon as possible so that we can run the most effective and efficient tryouts possible. Tryout fees are NON-REFUNDABLE.

STEP 2:

To attend any tryout, you will need to purchase a USAV tryout membership. First a SportEngine account must be created for the parent, then the parent can add a membership under “Household” for their daughter. STEP 3:

Please print & fill out the below Concussion & SCA Form as well the Medical Release form documents with you when you check in for your tryout. You will need to show these along with a copy of your USAV membership renewal confirmation to play!

ONCE ALL OF THE STEPS ABOVE HAVE BEEN COMPLETED/VERIFIED, PLEASE FILL OUT THE FORM BELOW TO REGISTER FOR TRYOUTS.

Once you have registered, you will be directed to the Online Registration Payment Portal.
